The episode opens with a shabbily dressed man asking Maomao to treat his sick child. Put off by his arrogance, she first tells him to go home if he has no money, then agrees to come after making him stamp a written contract. She brings along Ukyou, one of the menservants, and at the house finds a girl close to death. The mother had died after a miscarriage, and the father says he cannot have the family shrink any further. Asked what the girl had been fed, her older sister produces the sweet, which she had used to pad out the rice porridge. Maomao explains that the ergot in it can cause poor circulation in the hands and feet, paralysis, necrosis and hallucinations.
The girl recovers under Maomao's care. The older sister later turns up battered, and Maomao bluntly asks her about money, telling her that if she has none she could sell her younger sister to a brothel. The sister stands her ground, saying she does not want to end up living a life like mud, and Maomao, who says she prefers people who struggle and claw their way forward, decides to put in a word for her.
Elsewhere, information about a suspicious drug also reaches Jinshi, along with new details about a mysterious figure being gossiped about in town and a court lady holding a medical officer's post. The illustrated guides gathered from the fortress are brought not by Jinshi but by Basen. Maomao points out that the guides contain nothing about locusts, and one suggestion raised is that the missing material was placed in the coffin of an apothecary who died during an experiment in immortality. What she hears leads her to suspect that Suirei may know something, and the episode ends with word reaching Consort Ah-Duo and Suirei that Maomao is coming to see them.
